United States Military members may be eligible to receive a government marker. You will need to locate the discharge papers to prove your (or the veteran's) eligibility for government benefits. Keep the original paperwork for your records and only submit photocopies. When completing the form please be sure to check off 'flat granite' as the type of stone, and the emblem of belief is 01-Latin (Christian) Cross.

Holy Sepulchre Cemetery & Mausoleum is a Catholic cemetery and follows the rules and instructions as provided by the Vatican. The Congregation for the Doctrine of the Faith (Ad resurgendum cum Christo), prohibits the commingling, scattering or dividing of cremated remains. Cremation must be treated with the same reverence, dignity and respect an interment would receive.
To authorize the interment of someone in a grave you own, an Authorization of Interment form is required. Please select from the forms below and have same notarized. The original, raised seal document must be delivered or mailed to the office.
